Hospice Philosophy
Hospice care is a specialized care designed to provide support to you and your loved ones during a life-limiting illness. Hospice focuses on creating a natural and comfortable end-of-life experience for those confronted with a terminal condition. Through a range of palliative, medical, nursing, psychosocial, and spiritual care provided by an interdisciplinary team of experts, hospice seeks to manage symptoms and provide comfort when cure is no longer possible. Hospice care affirms life and views death as a natural process.

Level of Care
Routine Home Care is provided in the patient’s place of residence, in your private home, an assisted living community, a skilled nursing or long-term care facility, or another residential setting. Under routing care, your receive a regular visits from members of your care team, based upon your specific needs. As your needs change, team members adjust their visits to accommodate these changes and ensure optimal care.
Continuous Care is provided during periods of crisis to achieve palliation or management of acute medical symptoms. This care is provided in the patient’s home or facility where they live. In addition to being visited by the team members, the patient will receive a minimum of 8 hours a day care by a licensed nurse and hospice aid.
General Inpatient Care is care for pain control and symptoms management that cannot effectively be provided in other settings. It is usually a short-term nature and can be provided in a hospital, hospice unit or a long-term care facility.
Respite Care is a short-term inpatient care that is provided if a caregiver needs a rest/break from their care giving responsibilities. Under respite care, you may stay in a hospital, other inpatient center or nursing home for up to five days.
