Our Team

Your Physician or Medical Director
Your personal physician may continue to care for you and work in collaboration with the hospice team or you may choose to designate a hospice medical director as your primary physician.

Registered Nurses
Provide skilled care and support by understanding and explaining your illness and listening to your wants and need. Your hospice nurse is your case manager and will also provide education and hands on care, options to manage your illness and support your caregivers.

Certified hospice aides
Will assist you to make everyday tasks easier (bathing, grooming, dressing) while at the same time, helping you maintain your dignity and independence.

Social Workers
Provide support and counseling for you and those close to you and information to assist you with other practical concerns, such as alternative living arrangements, financial issues and referrals to other agencies. They also assist you in the preparation of a living will and with burial arrangements.

Chaplains
Initiates contact with patient and family to determine needs and provides spiritual support and counseling. They will listen and offer prayers, help you reflect on the things you find most meaningful in your life, and support you in healing relationships. Works as liason with a clergy of the individuals choice.

Volunteers
Share companionship with you and your loved ones. They can provide respite for caregivers by sitting with you while your family takes a break.

Beareavement counselors
Available to assist your loved ones as they grieve. Bereavement services are provided by masters-prepared, licensed counselors and specially trained volunteer, and include individual counseling, support groups, workshops and memorial services.
